What am I doing wrong? File and folder saving problems

I’m a bit anal, but I prefer my folders to be saved in a certain way.

What I want is something like this:

Media/tv/Show/Season #/Show - S##E## - Show name (if applicable)

Instead, after a while I get something like this http://i.imgur.com/D3UQ3gZ.jpg

Should just be 14 folders, but all the extra folders are episodes of shows I have downloaded.

Another thing, I keep getting empty folders in some of the original 14 shows folders, how would I go about automatically deleting those?

Separate your sorted and unsorted files. Sounds like Sonarr isn’t sorting files, you’ll have to check to logs to see why.
