Not finding torrent files

Sometimes I’ll have a missing episode. I’ll click the episode and instruct Sonarr to automatically search for it. Sometimes that fails. Then, when I got to one of the torrent sites I have listed under “indexers” and search for the episode myself, I find it. In the qualities I have set as preferred too (so it’s not that).

What’s happening?

Using KAT? Its probably unverified posts, turn on advanced options in Sonarr and allow unverified posts.
